Key Responsibilities\n

Your responsibilities as an LCSW will include:

• Conducting thorough clinical assessments to understand patients' emotional and mental health status, identifying their specific needs and concerns.

• Developing comprehensive treatment plans that utilize solution-focused interventions and evidence-based practices tailored to your patients' goals.

• Administering various therapeutic modalities such as Cognitive Behavioral Therapy (CBT) and Dialectical Behavior Therapy (DBT), ensuring that treatments are responsive to each client’s preferences.

• Maintaining meticulous clinical documentation that complies with HIPAA standards, ensuring confidentiality and integrity of patient information.

• Collaborating with a multidisciplinary treatment team, including psychiatrists and psychologists, to coordinate holistic care for patients, discussing insights and strategies in regular meetings.

• Providing ongoing education and emotional support to patients and their families regarding mental health issues, treatment processes, and available community resources.

• Staying current with the latest research, innovations, and best practices in mental health to continuously enhance the care you provide.

• Managing a full caseload efficiently, ensuring that patient treatment goals are met consistently, reflecting best practices in patient-centered care.

Patient Population / Client Focus\n

This role specifically focuses on providing compassionate care to hospice patients and their families, addressing a range of mental health challenges triggered by critical health conditions. You will work with individuals who may be experiencing grief, anxiety, depression, and other emotional distress associated with end-of-life issues. This requires sensitivity, empathy, and strong interpersonal skills to foster a safe space where patients and families can express their concerns and cope with their circumstances.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• A valid and active license to practice as a Licensed Clinical Social Worker (LCSW) in the state of New York.

• A minimum of 3 years of clinical experience, specifically in providing psychotherapy services.

• Demonstrated knowledge and application of solution-focused psychotherapy techniques.

Compensation and Benefits\n
