A full-time opportunity is available for an experienced Registered Nurse specializing in home health care. This 13-week contract position involves delivering compassionate, skilled nursing services to patients in their homes, supporting recovery and maintaining independence. Field-based work allows you to build meaningful connections with patients and their families while enjoying autonomy and a dynamic schedule.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Role Highlights:

• Daily travel within an assigned territory; mileage reimbursement provided
• Caseload expectation of 30 points per week, averaging 35 patient visits per day

Key Responsibilities:

• Perform thorough OASIS assessments and develop personalized care plans
• Deliver skilled nursing services such as wound care, IV therapy, medication administration, and chronic disease oversight
• Monitor patient progress, communicate changes to the interdisciplinary team, and coordinate with physicians as needed
• Educate patients and families about health management, medications, and preventive care strategies
• Document patient care and communication accurately and promptly within Home Care Home Base (HCHB)
• Collaborate closely with physical and occupational therapists, social workers, and home health aides to ensure integrated care delivery
• Participate in case conferences, staff meetings, and ongoing educational activities
• Ensure compliance with regulatory and organizational standards

Qualifications:

• Active RN licensure in the state (in good standing)
• One or more years of recent field-based Home Health experience
• Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance coverage

$2,057 - $2,176/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
