Key Responsibilities\n

As a Licensed Clinical Social Worker, your primary responsibilities will include:

• Conducting comprehensive psychosocial assessments to develop personalized treatment and discharge plans in coordination with the integrated healthcare team.

• Participating in community outreach by representing Redesign Counseling Hub at local meetings and committees aimed at addressing the social needs of our clients.

• Providing individual and group counseling sessions focused on crisis intervention, emotional support, and the treatment of behavioral issues.

• Collaborating with physicians and other healthcare professionals to communicate patient progress, identify critical needs, and troubleshoot challenges that may arise during treatment.

• Maintaining detailed and up-to-date clinical documentation, including progress notes and treatment plans, in compliance with departmental policies and standards.

Patient Population / Client Focus\n

Your role will involve working with a varied patient population, including individuals dealing with anxiety, depression, trauma, and other mental health challenges. Emphasizing a holistic approach, you will address both the psychological and social factors impacting patients' health, ensuring that they feel supported throughout their treatment journey.

Qualifications\n

To be considered for this position, candidates must meet the following qualifications:

• A Master’s degree in Social Work (MSW) with appropriate licensure as an LCSW.
