Work Environment

This position is based at our outpatient office, where NPs work onsite in a collaborative setting. Key Responsibilities

The Nurse Practitioner will undertake a range of responsibilities critical to patient care, including:

• Documenting patient encounters accurately within our EMR system to maintain compliance and improve care quality.
• Engaging with local and regional operations and clinical leadership to ensure that our goals for patient care and operational efficiency are met.
• Supporting our strategic initiatives focused on value-based care, aiming to enhance health outcomes while optimizing resource use.
• Creating and maintaining an excellent customer experience that fosters high levels of patient satisfaction and retention.
• Practicing ethically and adhering strictly to the Scope and Standards of Practice as well as Board Certification requirements.
• Coordination of care through collaboration with primary care physicians (PCPs) and interdisciplinary teams to ensure cohesive treatment plans are followed.
• Completing regulatory-required visits, including monthly, quarterly, and annual appointments to monitor patient progress effectively.
• Participating in family conferences and providing advanced care planning and education to empower families and caregivers.
• Addressing pharmacy recommendations and communicating with pharmacists regarding residents’ medication plans.
• Assessing and managing both chronic and acute conditions, delivering holistic and individualized treatment approaches.
• Providing health education to patients and families to foster better self-management of their health.
• Ordering and interpreting lab tests and diagnostic imaging to refine treatment plans as necessary.

Patient Population / Client Focus

Our focus is primarily on serving senior citizens and low-income individuals, providing them with access to necessary medical care and support. The Nurse Practitioner will interact with a diverse patient demographic, ensuring that all individuals receive tailored healthcare that addresses their unique needs and circumstances.

Qualifications

The ideal candidate will possess the following qualifications:

• An active DEA license and US work authorization.
• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• Current national certification from AANP or ANCC.
• An active, unrestricted state license to practice as a Nurse Practitioner.
• A genuine passion for serving the healthcare needs of senior populations and those in low-income situations.
• Strong adaptability in a flexible work environment and schedule.

Preferred Experience

Preferred candidates will have prior experience in outpatient settings, particularly with geriatric care or community health programs. Familiarity with collaborative care models and a track record of effective communication within multidisciplinary teams will be advantageous.

Compensation and Benefits
