Position Overview

The Special Care Behavioral Treatment Group is seeking a compassionate and dedicated full-time Nurse Practitioner (NP) to join our team in Rochester, New York. Key Responsibilities

• Conduct thorough assessments of patients in order to create and implement individualized care plans that address both mental and physical health needs.
• Facilitate hospital admissions, transfers, and discharges while ensuring appropriate coordination of inpatient and outpatient accommodations, including home-care arrangements as needed.
• Provide comprehensive counseling and education to patients and their caregivers, focusing on understanding disease processes, care management strategies, and lifestyle modifications.
• Perform phone triage for acute clinical issues, conducting assessments per protocol, and prescribing appropriate treatment to promote positive patient outcomes.
• Engage actively in discharge planning and coordinating continued care, directing and delegating tasks within the healthcare team to ensure seamless patient transitions.
• Collaborate with multidisciplinary teams, contributing to the development, implementation, and evaluation of standards of care and professional practice that align with organizational policies and regulatory requirements.

Patient Population / Client Focus

You will work with a diverse patient population, including individuals struggling with various behavioral health challenges, such as anxiety, depression, substance use disorders, and more. Qualifications

• Board-certified in Family Medicine or Internal Medicine.
• Possession of an active, unrestricted New York state nursing license.
• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
• Authorization to work in the U.S. without sponsorship.
• Strong commitment to patient-centered care and willingness to adapt to a flexible schedule to meet patient needs.
• Excellent communication and interpersonal skills, with a passion for providing exemplary customer service.
• Comfortable operating within an on-site clinic environment and engaging with patients from diverse backgrounds.

Preferred Experience

• Previous experience working in behavioral health or mental health settings.
• Proficient in utilizing electronic health record (EHR) systems for documentation and care coordination.
• Ability to work collaboratively with multidisciplinary teams for comprehensive patient care.
• Experience conducting telehealth assessments and consultations will be an asset.

Compensation and Benefits

The annual salary for this position ranges from $150,000 to $160,000 , depending on your experience and patient caseload.
