Work Environment

This position is fully onsite, allowing you to engage directly with patients in their homes, which fosters a strong rapport and understanding of each patient’s individual circumstances. You will collaborate closely with a diverse medical team, leveraging various digital health technologies and coordination platforms to ensure that patient care plans are executed effectively and efficiently.

Key Responsibilities

• Develop and implement comprehensive treatment plans based on scientific evidence, industry standards, and best practices tailored to each patient’s unique health profile.
• Conduct thorough assessments to diagnose and treat acute healthcare issues, including illnesses, infections, injuries, and chronic diseases.
• Prescribe appropriate medications, ensuring that all choices are made with consideration of efficacy, safety, and cost, in alignment with state regulations.
• Design interventions aimed at modifying patient behaviors associated with health risks, enhancing the overall quality of care.
• Educate patients and their families on managing both acute and chronic health conditions, providing personalized guidance and resources to empower them in their health journeys.
• Frequent evaluations of patients’ medical or psychological conditions will form an essential part of your responsibilities, ensuring their ongoing health needs are met.

Patient Population / Client Focus

You will serve a diverse clientele, focusing on patients of all ages who require personalized healthcare management in the home care environment. This could range from pediatric patients with chronic conditions to elderly individuals needing routine health checks. Qualifications

• Master's Degree in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program is required.
• Current and unrestricted Nurse Practitioner license in New York State is mandatory.
• National certification from AANP or ANCC is necessary for this role.
• Valid U.S. work authorization is required.
• Demonstrated ability to adapt to a flexible work environment and schedule as required by patient needs.

Preferred Experience

• Experience in home health care or community-based settings is highly valued.
• A strong understanding of chronic disease management and preventative care strategies.
• Experience with electronic health record (EHR) systems and telehealth tools is an asset.

Compensation and Benefits

Complete Care Healthcare Services offers a competitive salary range of USD 150,000 – 160,000 per year, depending on your caseload and experience level.
