Work Environment

The Nurse Practitioner role is onsite and involves direct patient interaction in various home settings. Our home care model allows NPs to develop meaningful relationships with clients, catering to their individual healthcare needs. Key Responsibilities

• Conduct comprehensive health assessments and develop tailored treatment plans that are effectively communicated to patients, families, and interdisciplinary teams.
• Provide health education to patients and their caregivers on managing chronic conditions and promoting wellness, ensuring they understand their treatment options and health goals.
• Order and interpret lab and diagnostic tests, using the results to adjust treatment protocols and improve patient outcomes accordingly.
• Accurately document all patient interactions and care provided within our EMR (Electronic Medical Record) system, maintaining compliance with legal and organizational standards.
• Engage in collaborative discussions with the operations and clinical leadership to align practice with strategic initiatives, focusing particularly on value-based care.
• Participate in regular training and continuing education workshops to stay current with best practices and innovations in geriatric healthcare.

Patient Population / Client Focus

This position primarily focuses on seniors across a spectrum of health conditions, including those in assisted living, skilled nursing facilities, and independent living communities. You will bring high-quality care directly to their homes, addressing both acute and chronic healthcare needs. Understanding the unique challenges faced by older adults is crucial for supporting their health and wellness journeys.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner Certificate Program.
• National certification from either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Active, unrestricted Nurse Practitioner license in New York State.
• Current US work authorization.
• Demonstrated ability to adapt to a flexible working environment, effectively manage time, and prioritize patient care duties.

Preferred Experience

• Previous experience in home health care or geriatric care settings will be advantageous.
• Familiarity with EMR software and experience in maintaining concise yet comprehensive electronic medical records.
• Ability to communicate effectively with multidisciplinary teams to enhance patient care processes.

Compensation and Benefits

Care Fair Healthcare Services, LLC offers a competitive salary range of $150,000 to $160,000 per year, based on your experience and caseload responsibilities. In addition, we provide a comprehensive benefits package that includes:
