Key Responsibilities

• Conduct comprehensive initial patient histories and perform detailed physical examinations, ensuring thorough assessment and understanding of each patient's health status.
• Develop, document, and communicate a clear plan of care tailored to individual patient needs, consistently following up to guarantee proper patient preparation and screening.
• Execute daily patient rounds to monitor and evaluate patient progress, fostering ongoing relationships and open lines of communication.
• Write orders for medications, laboratory tests, and diagnostic procedures, incorporating the latest evidence-based guidelines to ensure patient safety and effective treatment.
• Interpret laboratory and diagnostic test results, engaging with patients to explain findings and necessary next steps in care.
• Collaborate with attending physicians, nursing staff, residents, and interdisciplinary team members to optimize patient outcomes and care delivery.
• Document discharge summaries, prescriptions, and referrals for patients upon discharge, ensuring that all relevant information is accurately captured in patient charts.
• Provide comprehensive post-discharge care instructions, empowering patients with knowledge and resources for a smooth recovery.
• Fulfill related duties as required, contributing to a team-oriented approach to providing healthcare under the guidelines of the Americans with Disabilities Act.

Patient Population / Client Focus

Qualifications

• Master’s Degree in Nursing from an accredited Nurse Practitioner program.

• A collaborative practice agreement with established protocols filed with the NYS Department of Education or attestation as per New York's Nurse Practitioner Modernization Act.
