Work Environment

The Nurse Practitioner will operate primarily onsite, visiting patients in their homes as part of our urgent care services. This setting allows for a unique and enriched patient-provider relationship. Our practice hours are Monday to Friday, with flexible scheduling options from 8 am to 4 pm or 9 am to 5 pm, ensuring a good work-life balance for our team members. Key Responsibilities

• Conduct thorough assessments by taking comprehensive patient histories, performing physical examinations, and ordering necessary diagnostic tests.
• Diagnose and manage acute, episodic, and chronic health issues, tailoring care plans that align with the individual needs of each patient.
• Prescribe medications and develop treatment strategies, ensuring all prescriptions are appropriate and effective for the patient population served.
• Implement and adhere to patient management policies and procedures, optimizing care delivery within the practice.
• Coordinate referrals to specialists as needed, maintaining open communication with other healthcare providers to support comprehensive care for patients.
• Engage in continuous collaboration with a supervising physician and specialist colleagues to elevate the quality of patient care and patient outcomes.
• Educate patients and their families on health maintenance, disease prevention, and follow-up care to empower them in managing their health goals.
