Position Overview

One Medical Health Care Clinic is seeking a dedicated and experienced Nurse Practitioner to join our team in Henrietta, New York. Key Responsibilities

• Conduct thorough health assessments and screenings for students and their families, including routine check-ups and urgent care interventions.
• Diagnose a wide range of acute and chronic medical conditions, developing tailored management plans that promote preventive care and health education.
• Collaborate with local organizations and stakeholders to create and implement effective health education programs that empower families with knowledge about nutrition, exercise, and general wellness.
• Advocate for patients' rights and ensure they have equitable access to necessary healthcare services, providing culturally sensitive care that respects the diversity of our community.
• Oversee referrals and coordinate follow-up care for patients, assuring continuity and quality of healthcare.

Patient Population / Client Focus

As a Nurse Practitioner with One Medical Health Care Clinic, your primary focus will be the health and wellbeing of school-aged children and their families. You will play a crucial role in addressing health disparities and improving access to necessary healthcare resources while ensuring a welcoming environment for diverse populations.

Qualifications

• Master's or doctoral degree in Nursing from an accredited institution.
• Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by recognized credentials (ANCC or AANP).
• A current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Possession of a DEA number and NPI number.
• Demonstrated proficiency in clinical assessment and diagnostic techniques.
• Exceptional communication skills, capable of building rapport with a wide range of stakeholders, including parents, educators, and healthcare personnel.
• Ability to both work independently and collaborate seamlessly within a healthcare team.
• Familiarity with EHR systems and comfort in utilizing technology in patient management.
• A minimum of two years of clinical experience is required, with a preference for experience in primary care or community health settings.
