Work Environment

This position is primarily held onsite at our Home Care facility in Pittsford, NY, where you will work closely with clients in their homes and the community to execute our mission. The working schedule typically runs from 9:00 a.m. to 6:00 p.m., with the possibility of evening clinics extending until 8:00 p.m. Participation in weekend community outreach events, including health fairs, is also expected. You will be part of a committed team working in a supportive environment aimed at fostering health equity.

Key Responsibilities

• Conduct comprehensive health assessments for patients, developing individualized care plans based on their unique needs.
• Diagnose and manage a range of acute and chronic conditions, utilizing evidence-based practices to ensure patients receive the highest quality of care.
• Implement preventive care strategies and health promotion initiatives to educate individuals about maintaining wellness.
• Partner with community organizations to create and deliver targeted health education programs that address pressing health concerns.
• Ensure continuity of care by coordinating referrals and follow-up appointments, maintaining communication with patients and providers.
• Document patient encounters and care plans meticulously, adhering to regulatory standards and maintaining confidentiality.
• Actively engage in community outreach by participating in events that promote health awareness and provide health screenings.
• Stay informed on current community health trends and best practices to better serve our patient population.

Patient Population / Client Focus

The Family Nurse Practitioner will primarily work with diverse individuals and families, many of whom come from underserved backgrounds. Your role extends beyond clinical care; you will advocate for health equity and work to eliminate barriers that prevent individuals from accessing essential healthcare services. By building strong relationships with your patients, you will contribute to better health outcomes and improved community health statistics.

Qualifications

• A current certification as a Family Nurse Practitioner, with a valid state nursing license.
• A Master's degree in nursing from an accredited institution.
• A minimum of three years of clinical experience, ideally in community health or public health settings.
• Excellent communication and interpersonal skills, enabling effective collaboration with patients from all backgrounds.
• A commitment to social justice and health advocacy, particularly for marginalized groups.

Preferred Experience

Compensation and Benefits

Radiant Wellness Clinic offers a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications.
