Concierge Health Nurse Practitioner for Community-Focused Care

Position Overview

We are seeking a dedicated and compassionate Health Nurse Practitioner to join our team at One Medical Health Care Clinic located in Union Hill, New York. This full-time position offers a unique opportunity to provide high-quality, patient-centered care while engaging with the community through a concierge medicine model and house calls. You will work Monday to Friday, from 9 AM to 5 PM, focusing on holistic health approaches tailored to individual and family needs.

Work Environment

This onsite role allows you to interact directly with patients in various settings, including home visits, ensuring that you meet individuals in their own environments. Key Responsibilities

• Conduct thorough health assessments and screenings, utilizing both physical examinations and comprehensive patient histories to inform your clinical decisions.
• Diagnose and create management plans for acute and chronic conditions, creatively implementing preventive care strategies and health promotions tailored to the patient population.
• Coordinate with local community organizations to introduce and support health education initiatives, thereby fostering better health practices among residents.
• Provide culturally competent care, ensuring that you advocate for your patients’ rights and facilitate access to necessary healthcare services.
• Manage referrals, ensuring continuity of care by following up with patients post-visit to monitor progress and adapt care plans as needed.
• Maintain precise documentation of patient encounters and treatment plans in compliance with industry regulations and to uphold high standards of patient care.
• Engage actively in community outreach, participating in local health fairs and events to promote health awareness and educate the public.
• Stay informed about current health trends and evidence-based practices that may impact patient care and community health outcomes.

Patient Population / Client Focus

This role primarily focuses on individuals and families in the Union Hill community, including underserved populations who may face barriers in accessing quality healthcare services. Your ability to build trust and establish rapport with diverse groups will be crucial in delivering effective health interventions and educating patients about their health.

Qualifications

• Must possess a Master’s or Doctoral degree in Nursing from an accredited program.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized institutions, such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A valid, unrestricted Nurse Practitioner (APRN) license in New York State is required.
• DEA and NPI numbers are mandatory for prescription writing.

Preferred Experience

• A minimum of 2 years of clinical experience, with preference given to candidates with backgrounds in primary care or family medicine.
• Strong skills in clinical assessment, diagnosis, and the ability to adapt to varying patient needs.
• Demonstrated ability to communicate effectively with patients and healthcare colleagues from diverse backgrounds.
• Proficiency using electronic health records (EHR) for documenting patient interactions and managing care efficiently.

Compensation and Benefits

This position offers a competitive salary in the range of $150,000 to $160,000 per year, based on qualifications and experience.
