Position Overview

We are seeking a dedicated and compassionate Community Health Nurse Practitioner for a full-time position at our US Veterans Health Care Clinic located in Webster, New York. Work Environment

Key Responsibilities

• Develop, implement, and oversee community health initiatives and educational programs aimed at enhancing the overall well-being of residents and their families.
• Deliver culturally sensitive and comprehensive healthcare by conducting thorough assessments, developing personalized care plans, and advocating for patient rights and access to necessary services.
• Maintain meticulous medical records, ensuring compliance with federal and state regulations, and uphold the highest standards of patient confidentiality.
• Actively participate in health fairs, community outreach events, and screening programs, providing vital health information and resources to underserved populations.
• Stay informed about emerging health trends, evidence-based practices, and policy changes impacting community health.

Patient Population / Client Focus

This role primarily focuses on serving elderly patients, particularly veterans residing within our assisted living facility. You will work closely with individuals with diverse backgrounds and varying health conditions, addressing their unique needs and fostering a supportive healthcare environment.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited institution is required.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Possession of a current, unrestricted Nurse Practitioner (APRN) license in the state of New York.
• DEA# and NPI# are mandatory for this position.
• Strong clinical assessment and diagnostic capabilities are essential for effective patient care.

Preferred Experience

• A minimum of 2 years of clinical experience is required, with a strong preference for candidates who have prior experience in community health or public health settings.
• Proven interpersonal and communication skills, enabling you to effectively engage with a diverse patient population and collaborate with interdisciplinary teams.

Compensation and Benefits

The compensation for this role ranges from $150,000 to $160,000 per year, commensurate with experience and qualifications.
