Work Environment

In this role, you will be providing care directly to patients within their homes, allowing you to build meaningful relationships and create personalized treatment plans. Key Responsibilities

• Collaborate effectively with a team of physicians, nurses, and other healthcare professionals to deliver seamless care across various healthcare settings.
• Perform detailed patient assessments, including gathering medical, physical, and psycho-social histories, as well as executing thorough physical exams tailored to each patient's needs.
• Utilize evidence-based practices to conduct non-invasive preventive screenings, aiding in early detection and management of health issues.
• Develop and implement personalized differential diagnoses, taking into consideration comprehensive clinical findings and patient histories.
• Educate patients on health management strategies and develop tailored educational materials that promote understanding and compliance.
• Prescribe appropriate pharmacologic and non-pharmacologic therapies in accordance with established medical guidelines and standards of care.

Patient Population / Client Focus

This position primarily focuses on an adult patient population with a wide range of internal medicine concerns, including chronic illness management, preventive care, and acute health issues. As a Nurse Practitioner, you will play a crucial role in promoting health literacy, empowering patients to take charge of their health, and improving their overall quality of life.

Qualifications

• Master’s Degree in Nursing with a specialization as a Nurse Practitioner.
• Active and unrestricted Registered Nurse (RN) license in New York State.
