Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families, ensuring a thorough understanding of their medical history and current health status.
• Diagnose and manage a range of acute and chronic medical conditions, tailoring treatment plans to meet the specific needs of each patient.
• Engage in health promotion activities, including community outreach events and health fairs, to raise awareness and provide critical health screenings.
• Coordinate patient referrals and follow-up care to maintain continuity in treatment and ensure the highest standards of care.

Patient Population / Client Focus

Qualifications

• A Master’s degree in nursing from an accredited institution is required.
• Possession of an active state nursing license and current Nurse Practitioner certification.

Compensation and Benefits
