Position Overview

The Integrative Health and Wellness Clinic is actively seeking a dedicated and skilled Nurse Practitioner (NP) to join our exceptional team in Rochester, New York. This full-time position focuses on providing high-quality patient care within the environment of concierge medicine and house calls, ensuring personalized attention to our patients' health needs. The NP will operate under the guidance of our Chief Nurse Practitioner and the leadership of the CACC Medical Director, becoming an integral part of the Center for Advanced Cardiac Care (CACC) team.

This role operates primarily in a concierge medicine setting, where the Nurse Practitioner's services are offered directly to patients in their homes or in outpatient facilities. Key Responsibilities

As a Nurse Practitioner at our clinic, you will have a dynamic range of responsibilities, including but not limited to:

• Conducting thorough patient histories and performing both comprehensive and targeted physical examinations to assess health conditions.
• Ordering, interpreting, and analyzing diagnostic tests, enabling an accurate diagnosis and effective management of chronic and acute health concerns.
• Developing collaborative care plans, autonomously providing treatment, and facilitating patient care to enhance health outcomes.
• Reviewing and integrating internal and external records to inform clinical decisions.
• Formulating goal-directed and individualized care strategies, ensuring adherence to established clinical protocols for optimal health maintenance.
• Communicating openly with physicians to refine treatment plans based on evolving patient needs.
• Providing education and counseling to patients and caregivers about disease management and preventative care.
• Coordinating healthcare transitions by facilitating hospital admissions and collaborating with inpatient teams to ensure safe discharges.
• Participating actively in multidisciplinary discussions, including working closely with social workers, nutritionists, and other specialists to deliver holistic patient care.
• Documenting all interactions and care plans meticulously in electronic medical records to ensure continuity and accountability in patient care.
• Engaging in research protocols to continuously improve clinical practices through evidence-based approaches.
• Upholding the highest professional standards and accountability within the scope of Nurse Practitioner practice.
• Assuming additional responsibilities as assigned, reflecting our commitment to flexible and responsive care delivery.

Patient Population / Client Focus

This Nurse Practitioner role primarily serves adult patients within the cardiology domain, with a focus on those experiencing complex cardiac conditions such as advanced heart failure, pulmonary arterial hypertension, and individuals post-heart transplant. The NP will utilize clinical expertise to guide patients considering advanced therapies and treatments, including outpatient intravenous medication support and mechanical circulatory support systems.

Qualifications

The successful candidate will possess the following:

• A Master’s Degree in Nursing, with specialized NP training in adult cardiac care.
• A minimum of 2 years of clinical experience in a hospital or outpatient setting, with a preferred focus on cardiology.
• Current Registered Professional Nurse and Nurse Practitioner licenses in New York State.
• Board certification as a Nurse Practitioner.
• Current certifications in Basic Life Support (BCLS), Advanced Cardiovascular Life Support (ACLS), and DEA registration.
• An understanding of electronic medical records and familiarity with computer programs such as Microsoft Office Suite.

Preferred Experience

• Experience managing the complexities of patients recovering from advanced cardiac interventions.
• Proficiency in caring for patients undergoing treatments that include outpatient intravenous therapies and mechanical support devices.

Compensation and Benefits

We offer a competitive salary range of $120,000 - $220,000 annually, based on your skills and experience.
