Work Environment

This position is primarily onsite, allowing you to work directly in our patients' homes where you will deliver care tailored to their unique needs. Key Responsibilities

• Conduct thorough assessments of patients’ acute and chronic medical conditions to determine immediate healthcare needs.
• Diagnose medical issues through evidence-based practices and develop individualized treatment plans that address patients' needs.
• Order relevant diagnostic tests and interpret their results to guide clinical decision-making.
• Prescribe necessary medications and therapies, monitoring effectiveness and adjusting treatments as appropriate.
• Provide ongoing education to patients and their families regarding disease prevention, health promotion strategies, and self-care techniques.
• Work collaboratively with physicians and healthcare professionals to manage and coordinate comprehensive patient care.
• Maintain accurate and up-to-date medical records in line with regulatory standards while ensuring confidentiality.
• Engage in quality improvement measures, utilizing patient feedback and outcomes data to enhance service delivery.
• Stay informed about the latest healthcare trends, best practices, and regulatory updates affecting patient care.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve adult patients with a range of health conditions, including cardiovascular diseases, chronic illnesses, and general medical issues requiring ongoing management. Your role will be pivotal in restoring and sustaining the health of patients in the home care setting, allowing for personalized interactions that significantly contribute to improved patient satisfaction and outcomes.

Qualifications

• Current Nurse Practitioner certification.
• Active nursing license in New York State.
• Bachelor's degree in nursing from an accredited institution.
• Completion of a Master's degree in nursing, focusing on advanced practice.

Preferred Experience

• A minimum of 3-5 years of clinical experience in primary care or a specialty relevant to home healthcare.
• Demonstrated expertise in chronic disease management, patient education, and effective communication.

Compensation and Benefits

We offer a competitive salary range of $120,000 to $220,000 annually, commensurate with experience and qualifications.
