Advanced Cardiology PC is seeking dedicated and compassionate Nurse Practitioners to join our team in North Greece, New York. Work Environment

The Nurse Practitioner will work onsite in a hospital setting, collaborating with a diverse care team that includes physicians, physician assistants, medical assistants, and dedicated support staff like medical scribes and care managers. Key Responsibilities

• Deliver compassionate and exceptional primary care to patients, conducting thorough evaluations and developing comprehensive care plans.
• Perform both routine and acute office visits, addressing diverse health issues appropriately and effectively.
• Administer annual wellness visits and health risk assessments, focusing on holistic health and precise documentation.
• Coordinate care with other healthcare providers, specialists, and testing facilities to ensure seamless patient experience and continuity of care.
• Lead population health initiatives, such as regular screenings and preventive care measures, to enhance community health outcomes.
• Assist in triaging patient phone calls and outreach efforts, offering guidance as needed.
• Educate patients and their families about health conditions, treatments, and care plans, empowering them to actively participate in their health journey.
• Participate in community outreach and promotional activities to raise awareness of our services and mission.
• Conduct home visits for patients who are unable to come to the clinic, ensuring they receive comprehensive care.
• Collaborate closely with operational leadership and report directly to the Medical Director.

Patient Population / Client Focus

Our primary focus is on older adults enrolled in Medicare. We are dedicated to addressing the unique health challenges faced by this demographic, ensuring they receive personalized, compassionate care that emphasizes prevention, management of chronic conditions, and overall well-being.

Qualifications

• Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner certificate program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner, accredited by AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license and valid DEA license.
• Proven experience in primary care, particularly in internal medicine, geriatrics, or family medicine preferred.
• Solid understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
• A strong commitment to teamwork, collaboration, and a patient-centered approach.

Preferred Experience

• Previous experience working within a value-based care model.
• Familiarity with electronic health records and other digital tools for patient management.
• Active involvement in quality improvement initiatives to enhance patient care standards.

Compensation and Benefits

This full-time position offers a competitive salary range between USD 120,000 and 160,000 annually, depending on experience and qualifications.
