Lead Nurse Practitioner in Internal Medicine | Onsite at School Health Center

Position Overview

CareMed Primary and Urgent Care is seeking a dedicated and experienced Nurse Practitioner to join our team in North Greece, New York. This full-time position focuses on delivering high-quality internal medicine care to our student population at a local school health center. As a key member of our healthcare team, you will utilize your clinical expertise to provide comprehensive medical care, ensure continuity of care, and improve patient health outcomes.

Work Environment

This position is primarily onsite at a school health facility, where you will create a safe and supportive environment for students to receive medical care. Each day, you will be interacting with young patients in a friendly atmosphere, ensuring they feel comfortable while seeking treatment. Our school setting is equipped with the latest medical technology and electronic health record (EHR) systems, allowing you to efficiently manage patient information and streamline workflow.

Key Responsibilities

• Collaborate with physicians, nurses, and other healthcare professionals to provide comprehensive care across the continuum.
• Conduct thorough patient assessments, including obtaining detailed medical, physical, and psycho-social histories while performing comprehensive physical examinations based on the patient's age and health history.
• Implement non-invasive preventive screening procedures tailored to students’ age and history, ensuring early detection of health issues.
• Formulate differential diagnoses based on patient assessments, and develop tailored patient education and treatment plans.
• Prescribe and manage pharmacologic and non-pharmacologic therapies aligned with established treatment protocols for common conditions affecting the student population.

Patient Population / Client Focus

Your primary patient base will consist of students from diverse backgrounds within the local community. You will address a range of health concerns, from routine check-ups and vaccinations to managing chronic conditions and acute illnesses. Your empathetic approach and ability to communicate effectively with children and adolescents will play a crucial role in fostering trust and comfort in seeking healthcare services.

Qualifications

• Master’s Degree in Nursing as a Nurse Practitioner.
• Active Registered Nurse (RN) license in New York State.
• Strong skills in patient assessment, diagnosis, and developing treatment plans.
• Proficiency in using electronic health records and other medical technologies.
• Excellent communication skills for educating students and advocating for their health needs.

Preferred Experience

We are looking for candidates with at least three to four years of relevant clinical experience, ideally in a primary care or school health setting. Familiarity with adolescent health issues, developmental milestones, and the unique challenges faced by school-age children will be advantageous. Experience in providing preventive care, health education, and managing chronic conditions will enhance your effectiveness in this role.

Compensation and Benefits

We offer a competitive salary range of USD 150,000 to 160,000 per year. In addition to an attractive base salary, CareMed provides a comprehensive benefits package, including:
