This full-time role offers an annual salary range of USD 120,000 to 160,000, commensurate with experience and qualifications. Work Environment

This position is situated in a collaborative outpatient setting, where our Nurse Practitioners work closely with a multidisciplinary care team. You will be supported by physicians, medical assistants, dedicated medical scribes, registered nurses, and care managers to ensure seamless delivery of care. We specialize in concierge medicine and house calls, bringing healthcare to the communities we serve, and partnering with a network of elite specialists and hospitals to coordinate comprehensive care for our patients.

Key Responsibilities

• Deliver exceptional primary care tailored to the unique needs of older adults.
• Conduct thorough office visits to address both routine health maintenance and acute medical issues.
• Perform Annual wellness visits and comprehensive health risk assessments, emphasizing accurate documentation and a holistic view of patient health.
• Coordinate care with other healthcare providers, specialists, and testing facilities, ensuring continuity of care.
• Lead population health initiatives to meet screening and preventive care benchmarks, such as ensuring compliance with evidence-based screening for breast cancer.
• Assist the care team with phone triage, outreach, and patient follow-up.
• Educate patients about their health conditions, treatment options, and care management plans to foster positive health outcomes.
• Engage in promotional activities aimed at community outreach and education.
• Conduct home visits for patients requiring in-home healthcare.
• Fulfill additional responsibilities as assigned by the Medical Director, collaborating closely with operational leadership.

Patient Population / Client Focus

Our primary focus is on older adults enrolled in Medicare, many of whom face complex health challenges and co-morbidities. We are dedicated to providing personalized care that addresses both physical and emotional needs, aiming to enhance their quality of life and functional capabilities. Our approach not only targets immediate healthcare needs but also emphasizes preventive care to reduce the risk of hospitalization.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or a graduate from an NP certificate program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• Current DEA license to prescribe medications.
• Proven experience in primary care settings, with a focus on internal medicine, geriatrics, or family medicine.
• Understanding of HCC documentation, ICD-10 coding, and health risk assessment protocols.
• A strong desire to work collaboratively within a team-oriented environment, contributing to patient-centered outcomes.

Preferred Experience

• Previous experience in concierge medicine or house call practices is a plus.
• Familiarity with electronic health records (EHR) systems, particularly those focused on patient-centered care workflows.
• Demonstrated ability to manage patient caseload effectively while maintaining a high standard of documentation and oversight.

Compensation and Benefits

We offer a competitive salary in the range of USD 120,000 to 160,000 annually, alongside a comprehensive benefits package.
