Internal Medicine Nurse Practitioner/Physician Assistant - Onsite in Assisted Living Facility

Work Environment

The work setting is onsite at a progressive assisted living facility located in Rochester, New York. The facility is equipped with modern healthcare amenities that promote a home-like environment for our residents. You will be working alongside a well-established team of healthcare professionals, including physicians, nursing staff, and support personnel, all dedicated to providing comprehensive care tailored to each resident’s needs.

Key Responsibilities

• Conduct thorough assessments of patients, including detailed medical histories, physical examinations, and diagnostic evaluations to identify health concerns.
• Develop and implement individualized patient management plans in collaboration with physicians and other healthcare providers, ensuring continuity of care.
• Provide education to patients and their families regarding disease management, treatment plans, and preventive health measures.
• Perform follow-up visits and adjustments to treatment plans as necessary, monitoring patient progress and responding to changes in their condition.
• Utilize electronic health record (EHR) systems to document patient interactions, management plans, and outcomes accurately and efficiently.
• Engage in interdisciplinary meetings and case discussions to ensure comprehensive care delivery across various specialties.
• Participate in quality improvement initiatives and contribute to the development of best practices within the facility.

Patient Population / Client Focus

This role focuses on a diverse population of residents in assisted living, many of whom may have chronic illnesses and complex health needs. You will work with elderly patients, ensuring that their care is not only effective but also compassionate and sensitive to their unique situations. Your expertise in internal medicine will contribute significantly to enhancing their quality of life.

Qualifications

• A Bachelor’s Degree in Nursing or a related field from an accredited program.
• A valid and current Nurse Practitioner or Physician Assistant license in the state of New York.
• Certification as a Nurse Practitioner or Physician Assistant, with preference given to candidates who have passed the Physician Assistant National Certifying Examination.
• Minimum of 2 years of clinical experience in an ambulatory care setting, preferably in internal medicine or geriatric care.

Preferred Experience

• Experience working in assisted living facilities or similar healthcare environments.
• Familiarity with EHR systems such as Epic or similar software for patient documentation.
• Strong communication skills and ability to work effectively within a multidisciplinary team.
• Experience in developing and implementing patient care plans and involvement in quality assurance activities.

Compensation and Benefits

The successful candidate will receive a competitive annual salary in the range of USD 150,000 to 160,000, commensurate with experience and qualifications.
