Key Responsibilities

• Collaborate with physicians and interdisciplinary healthcare providers to create and implement individualized care plans based on comprehensive assessments of patient health.
• Conduct thorough evaluations by obtaining and analyzing patients' medical histories, including physical exams tailored to their age and health status.
• Utilize evidence-based guidelines to perform preventive health screening procedures and identify any health risks or conditions that need to be addressed.
• Formulate differential diagnoses grounded in physical assessments and clinical evaluations, ensuring that all potential conditions are considered.
• Design and execute patient education plans that empower individuals to manage their health effectively and understand their treatment options.
• Prescribe and manage both pharmacologic and non-pharmacologic therapies, offering patients a holistic array of treatment options.
• Monitor patient progress, adjusting treatment plans as needed to ensure optimal health outcomes and adherence to established care standards.

Patient Population / Client Focus

The nurse practitioner in this role will cater to a diverse patient population that spans various age groups, from pediatric to geriatric patients. You will be tasked with addressing both acute health issues and managing chronic illnesses, thereby contributing significantly to enhancing the overall quality of life for your patients. Qualifications

• Master’s Degree in Nursing with a focus on Nurse Practitioner studies.
• Current and valid Registered Nurse (RN) license in the state of New York.
• Demonstrated clinical expertise with a minimum of 3 years of relevant experience in a healthcare setting.
• Proficient in utilizing medical technology and EHR systems to enhance patient care.

Preferred Experience

Compensation and Benefits
