This role is located in our outpatient office in Henrietta, NY, where you will work within a collaborative team composed of physicians, nurse practitioners, physician assistants, dedicated medical scribes, registered nurses, and care managers. Key Responsibilities

As an Adult-Gerontology Nurse Practitioner, your core responsibilities will include:

• Delivering exceptional primary care services, including conducting thorough office visits for routine check-ups and acute health issues.
• Administering annual wellness visits and health risk assessments, emphasizing holistic health management and meticulous documentation.
• Coordinating patient care with other healthcare providers, specialists, testing facilities, and community agencies.
• Leading population health initiatives, ensuring all eligible patients receive essential screenings and preventative care measures.
• Supporting the care team through phone triage, patient outreach, and follow-ups, enhancing continuity of care.
• Educating patients on their health conditions and treatment plans, empowering them with knowledge to manage their health actively.
• Participating in promotional activities that foster community engagement and educate the public about our services.
• Conducting home visits when necessary to ensure patient-centered care is accessible, especially for those with mobility challenges.
• Collaborating closely with the Medical Director and operational leadership to align on organizational goals and patient care strategies.

Patient Population / Client Focus

In this role, you will focus on a diverse patient population primarily comprised of older adults on Medicare. Our practice is dedicated to meeting the unique healthcare needs of this demographic, prioritizing preventative care, chronic disease management, and promoting healthy lifestyles. By building meaningful patient relationships, you will enhance trust and encourage active participation in their care plans, contributing significantly to improved patient outcomes.

Qualifications

The ideal candidate for the Adult-Gerontology Nurse Practitioner position must possess the following qualifications:

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• An active, non-probationary state Nurse Practitioner license.
• An active DEA license for prescribing controlled substances.
• Experience in primary care, geriatrics, internal medicine, or family medicine is essential.
• Strong understanding of care documentation and coding, including HCC (Hierarchical Condition Categories), ICD-10 (International Classification of Diseases), and Health Risk Assessments (HRAs).
• A passion for teamwork and a commitment to collaborative care.

Preferred Experience

While not mandatory, preference will be given to candidates who have prior experience in value-based care models and have led population health initiatives. Additionally, strong communication skills and previous experience in community-focused healthcare settings will be highly regarded.

Compensation and Benefits

This full-time position offers a competitive salary range of USD 120,000 - 160,000 annually.
