The Nurse Practitioner will work onsite at a school facility in Penfield, New York. Key Responsibilities

• Conduct thorough health assessments, including detailed health histories and physical examinations, to ascertain patients’ medical needs.
• Diagnose and manage a wide range of acute and chronic illnesses, developing individualized treatment plans tailored to each patient’s requirements.
• Order, interpret, and analyze diagnostic tests such as laboratory results and radiological studies to monitor health status and adjust care accordingly.
• Prescribe medications and recommend non-pharmacological treatments, ensuring adherence to evidence-based practices within the Nurse Practitioner scope of practice in New York.
• Assess the effectiveness of treatment plans, adjusting interventions as necessary to achieve optimal patient outcomes.
• Provide patient-centered health education and wellness promotion, guiding patients and their families in understanding their health conditions and treatment strategies.
• Engage in ongoing professional development through participation in team meetings, case reviews, and continuing education opportunities to maintain a high standard of patient care.

Patient Population / Client Focus

This role primarily focuses on adults requiring comprehensive internal medicine care. The Nurse Practitioner will manage diverse health needs, addressing both chronic conditions, such as diabetes and hypertension, and acute illnesses like infections or injuries. Through focused health promotion and education, you will play a vital part in empowering patients to take charge of their health and making informed decisions about their care.

Qualifications

• A Master’s degree in Nursing, qualifying you to practice as a Nurse Practitioner.
• Current New York State Registered Professional Nurse and Nurse Practitioner licenses.
• Must hold NP Board Certification and certifications in Basic Life Support (BCLS) and Advanced Cardiac Life Support (ACLS).
• Exceptional clinical skills along with strong verbal and written communication abilities.

Preferred Experience

• A minimum of three years of experience as a Nurse Practitioner in an internal medicine setting, working closely with adults to manage their health needs.

Compensation and Benefits
