Key Responsibilities

• Conduct thorough patient assessments through interviews and physical examinations to efficiently identify health concerns.
• Develop and implement comprehensive patient management plans tailored to individual health issues and lifestyle requirements.
• Collaborate with physicians and interdisciplinary teams to address both short-term and long-term health management strategies.
• Maintain accurate patient records by documenting medical histories, treatment plans, and follow-up care.
• Monitor patient responses to treatment and adjust plans as necessary to ensure optimal health outcomes.
• Educate patients and their families about health conditions, treatment options, and preventive care strategies.

Patient Population / Client Focus

Your primary focus will be on patients requiring internal medicine services, ranging from adults to seniors experiencing chronic conditions to acute illnesses. Qualifications

• Graduate from an accredited Nurse Practitioner or Physician Assistant program.
• Current licensure to practice as a Nurse Practitioner or Physician Assistant in New York.
• Completion of the Physician Assistant National Certifying Examination (or equivalent certification for NPs).
• A minimum of 2 years of experience in an ambulatory care setting preferred.

Compensation and Benefits
