Position Overview

Pulse Point Internal Medicine is seeking a dedicated and compassionate Nurse Practitioner to join our dynamic healthcare team in North Chili, New York. This full-time role will involve providing high-quality primary care to Medicare-aged adults in a home care setting. The Nurse Practitioner will address both routine health concerns and acute medical issues, while fostering strong patient relationships that enhance care continuity and patient satisfaction.

Work Environment

The Nurse Practitioner will work onsite within our outpatient home care facility in North Chili, NY, which provides a community-focused atmosphere. This position is part of a close-knit care team comprised of physicians, nurse practitioners or physician assistants, medical assistants, a dedicated medical scribe, and registered nurses, all dedicated to enhancing patient experiences. Each team member plays a vital role in ensuring that our patients receive the highest standard of primary care in a supportive and collaborative environment.

Key Responsibilities

• Provide exceptional primary care services, conducting thorough assessments and creating comprehensive management plans for patients.
• Perform office visits for routine check-ups and acute medical issues, ensuring patient comfort and thorough care.
• Administer annual wellness visits and undertake health risk assessments, focusing on a holistic view of health and detailed documentation.
• Coordinate care plans with other healthcare providers and specialists, ensuring seamless communication and high-quality patient care.
• Lead initiatives in population health management, ensuring all eligible patients receive evidence-based preventive screenings and care.
• Assist the care team with phone triage and proactive patient outreach to enhance health monitoring.
• Educate patients regarding their health conditions, treatment options, and preventive care strategies to empower them in managing their health.
• Participate in community promotional activities, representing the organization's mission and values.
• Conduct home visits as needed, allowing for personalized and direct patient care when circumstances require it.
• Work closely with the Medical Director and operational leadership to align with organizational goals and contribute to quality improvement initiatives.

Patient Population / Client Focus

The primary focus of this role is on older adults enrolled in Medicare. Our innovative and patient-centered care model seeks to support this demographic by placing an emphasis on preventive health, chronic disease management, and maintaining independence through tailored health programs.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active and non-probationary Nurse Practitioner license from the State of New York.
• Current DEA license.
• Experience in primary care settings such as internal medicine, geriatrics, or family medicine.

Preferred Experience

• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs) to enhance patient care and organizational efficiency.
• A genuine enthusiasm for teamwork, demonstrating the desire to collaborate across various disciplines within healthcare.

Compensation and Benefits

Pulse Point Internal Medicine offers a competitive salary ranging from USD 180,000 to 200,000 annually, with opportunities for further compensation based on experience and expertise. Our benefits package includes:
