Work Environment

This full-time position allows our Family Nurse Practitioner to operate onsite, providing services directly in patients' homes. Key Responsibilities

• Conduct thorough initial patient screenings, including comprehensive medical histories and physical examinations to establish a baseline for ongoing care.
• Maintain up-to-date electronic patient records during each appointment, ensuring all information is accurate and reflective of patient status and treatment plans.
• Analyze laboratory and diagnostic test results, determining the necessity for follow-up appointments or additional therapeutic interventions to optimize patient health.
• Deliver compassionate and effective care while meeting company productivity metrics, ensuring patient needs are prioritized at all times.
• Act as an educational resource for patients, providing ongoing counseling and support regarding management of chronic conditions and encouraging preventive health behaviors.
• Develop tailored treatment plans that utilize harm reduction techniques to empower patients on their journey toward health and wellness.

Patient Population / Client Focus

In this role, you will primarily serve families and individuals with varying health needs, particularly those who may have limited access to healthcare services or face barriers to traditional medical care. Qualifications

• A Master’s degree in Nursing from an accredited institution is required.

Compensation and Benefits

This position offers a competitive annual salary ranging from $150,000 to $160,000, reflecting your experience and skill level.
