Work Environment

This role will be performed onsite, primarily within the Home Care setting, allowing you to engage directly with patients in their environments. The typical work schedule will be from 9:00 a.m. to 6:00 p.m., with the possibility of evening clinics extending until 8:00 p.m. Flexibility may be required for community health outreach events that may occur on weekends. The average workweek totals approximately 40 hours.

Key Responsibilities

• Conduct thorough health assessments and screenings to identify individual and family healthcare needs within the community.
• Diagnose, manage, and treat acute and chronic medical conditions, emphasizing preventive care and health promotion.
• Design and implement health education programs in collaboration with community organizations to enhance public health awareness.
• Deliver culturally sensitive healthcare while advocating for patients' rights and ensuring access to necessary medical services.
• Coordinate patient referrals and follow-up care to guarantee continuity and quality of care.
• Maintain accurate and detailed medical records in compliance with healthcare regulations and standards.

Qualifications

Compensation and Benefits
