Work Environment

This position is based onsite at our hospital facility in Rochester, providing the unique opportunity to work directly with patients in a supportive and multidisciplinary team. The schedule is Monday through Friday from 9 AM to 5 PM, allowing for a balanced work-life dynamic while serving the community.

Key Responsibilities

• Conduct thorough health assessments and screenings, evaluating both acute and chronic health conditions for patients across the lifespan.
• Diagnose medical conditions, prescribe treatments, and develop tailored care plans that emphasize preventive care and health promotion.
• Engage with local community organizations to design and execute health education programs that inform and empower patients about their health.
• Advocate for patients, providing guidance on their rights and access to healthcare services, ensuring patient-centered care.
• Coordinate referrals to specialists and follow up with patients to maintain continuity of care, thereby enhancing patient outcomes.
• Record detailed medical histories and treatment plans, ensuring compliance with regulatory standards and contributing to quality assurance processes.
• Participate actively in community health events and health fairs, offering education and health screenings to foster public awareness.
• Stay informed about emerging community health trends and evidence-based practices to continually improve care delivery.

Patient Population / Client Focus

Our Nurse Practitioner will engage with a diverse patient population, ranging from children to seniors, each with unique health challenges and needs. This role emphasizes providing culturally sensitive care to address disparities in healthcare access, aligning with our mission to serve underserved communities effectively.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited institution.
• Current certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) through recognized accrediting bodies such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• A valid, unrestricted Nurse Practitioner (APRN) license in New York State is required.
• Possession of a DEA number and NPI number.
• Strong clinical assessment and diagnostic capabilities to foster effective patient care.
• Proven ability to communicate clearly and empathetically with patients from various backgrounds.

Preferred Experience

• A minimum of 2 years of clinical experience is required, particularly in primary care or family medicine settings.
• Experience working in community health or public health settings is highly preferred, enriching the ability to meet the needs of our patient population.

Compensation and Benefits

• The annual salary ranges from USD 150,000 to 160,000, based on experience and qualifications.
