Work Environment

As a Nurse Practitioner in our Home Care facility, you will find a team-oriented environment dedicated to fostering professional growth. Our operational hours generally span from 9:00 a.m. to 6:00 p.m., with some evening clinics extending until 8:00 p.m. Flexibility is key, as participation in community outreach events or health fairs on weekends may be necessary to maximize community engagement. The average workweek totals approximately 40 hours, ensuring a balanced approach to your professional and personal life.

Key Responsibilities

• Perform comprehensive health assessments and health screenings for diverse individuals and families, ensuring a thorough understanding of their medical history.
• Diagnose and manage a range of acute and chronic medical conditions while providing preventive care and health promotion strategies.
• Collaborate proactively with community organizations and stakeholders to design and implement impactful health education programs that resonate with community needs.
• Deliver culturally sensitive care, consistently advocating for patients’ rights and their access to necessary healthcare services.
• Coordinate patient referrals and facilitate follow-up care to guarantee continuity of healthcare services.
• Maintain meticulous and compliant medical records, adhering to established regulatory standards and ensuring patient confidentiality.
• Engage in community outreach initiatives and health fairs, conducting health screenings and educating the public on health matters.
• Stay abreast of the latest community health trends, best practices, and relevant health policies to provide the most effective care.

Qualifications

• A current and valid certification as a Nurse Practitioner.

Compensation and Benefits

Bright Health Medical Center offers a competitive salary range of $180,000 to $200,000 annually, commensurate with your experience and qualifications.
