Work Environment

As a Family Nurse Practitioner at Blissful Nurse Specialists, you will work on-site in a home care setting, providing services directly to patients in their homes. This position combines clinical excellence with a commitment to community service, allowing you to develop meaningful connections with patients and their families. Our office culture is collaborative and supportive, fostering an environment where ideas and solutions are shared to enhance patient care. You will work approximately 40 hours per week, with a schedule that generally ranges from 9:00 a.m. to 6:00 p.m., including occasional evening clinics until 8:00 p.m. Engagement in community outreach events and health fairs during weekends may also be required.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families, identifying their unique health concerns and needs.
• Diagnose both acute and chronic medical conditions, creating and managing personalized treatment plans that incorporate preventive care and health promotion strategies.
• Collaborate with local community organizations and stakeholders to develop and implement health education programs, focusing on wellness and disease prevention.
• Provide culturally sensitive and patient-centered care, advocating for the rights and access to healthcare services for all patients.
• Coordinate referrals and ensure follow-up care processes to maintain continuity of care for every patient.
• Maintain up-to-date and accurate medical records in compliance with healthcare regulations.
• Engage in community outreach initiatives to raise health awareness and provide essential health screenings.
• Stay informed about the latest community health trends, evidence-based practices, and relevant healthcare policies to ensure optimal patient care.

Patient Population / Client Focus

This role primarily focuses on serving diverse patient populations in the Henrietta area, including individuals and families from various socioeconomic backgrounds. You will work closely with underserved communities, aiming to overcome barriers to healthcare access and improving overall health outcomes. Your ability to build rapport with patients from different cultural backgrounds will be crucial to this role’s success.

Qualifications

• Current certification as a Nurse Practitioner, with an active state nursing license.
• A Master's degree in nursing from an accredited institution.
• Minimum of 3-5 years of clinical experience, preferably in community health or public health settings.
• Strong interpersonal and communication skills, with proven ability to effectively engage with a diverse patient population.
• A commitment to social justice, health equity, and advocacy for vulnerable communities.

Preferred Experience

Compensation and Benefits

Blissful Nurse Specialists offers a competitive annual salary ranging from $180,000 to $200,000, commensurate with your experience and qualifications.
