Work Environment

This role is based in North Chili, NY, where our clinic operates under a concierge model, allowing you the flexibility to provide care both in the office and through house calls. Your workweek is approximately 40 hours, typically scheduled from 9:00 a.m. to 6:00 p.m., with occasional evening clinics and community outreach events during weekends. By collaborating with community organizations, you will engage in health education initiatives that significantly impact the health of our community.

Key Responsibilities

• Perform comprehensive health assessments, including thorough physical examinations and screenings to identify health concerns in individuals and families.
• Diagnose, treat, and manage both acute and chronic medical conditions, ensuring that patient care includes preventive steps and health promotion strategies tailored to community needs.
• Collaborate actively with local community organizations and stakeholders to develop and implement effective health education programs that enhance community health literacy.
• Advocate for the rights and access of patients to healthcare services, ensuring culturally sensitive care that recognizes the diverse backgrounds of our patient population.
• Coordinate referrals and follow-up care to provide a seamless experience for patients, ensuring continuity in their treatment plans and health journeys.
• Maintain accurate and detailed medical records that comply with regulatory standards, utilizing appropriate electronic health record (EHR) systems.

Compensation and Benefits

Jackson Medical Center offers a competitive salary range of $180,000 to $200,000 per year, commensurate with your experience and qualifications.
