Work Environment

This position is based onsite at our state-of-the-art assisted living facility in Rochester, NY, which provides a supportive and engaging atmosphere for both staff and residents. Key Responsibilities

• Conduct comprehensive health assessments, including physical examinations and patient histories, to develop and implement individualized care plans.
• Manage acute medical situations and exacerbations of chronic conditions for our residents, utilizing appropriate testing, diagnostics, and interventions to address their healthcare needs.
• Oversee patients during short-term sub-acute rehabilitation episodes, monitoring progress and adjusting care plans as necessary.
• Ensure meticulous documentation of patient information, utilizing ICD-10 coding and electronic medical records to maintain accurate health data.
• Communicate effectively and collaborate with the interdisciplinary care team, including physicians, nurses, and support staff, to coordinate patient care and optimize health outcomes.
• Engage in advanced illness and end-of-life care planning discussions, providing compassionate support to patients and their families.
• Provide education and counseling to patients and caregivers regarding health management, chronic disease prevention, and lifestyle choices that promote well-being.

Patient Population / Client Focus

The Nurse Practitioner will primarily work with an elderly population residing in our assisted living facility, many of whom have complex medical conditions, behavioral health issues, and social needs. Our focus is on creating personalized care plans that respect the dignity and preferences of our residents while addressing their medical and emotional well-being. The goal is to empower our patients to lead healthier lives, nurturing not just their physical health but also their emotional and psychosocial needs.

Qualifications

• Current certification as a Nurse Practitioner, with an active state nursing license in New York.
• A Master’s degree in nursing from an accredited institution.
• A minimum of 3 years of clinical experience, with proven capabilities in a hospital, assisted living, or skilled nursing setting.

Preferred Experience

• Extensive clinical experience in long-term care, acute care, emergency medicine, or urgent care settings.
• Proficiency in managing patients with complex behavioral and functional needs, making strong communication and decision-making skills essential.
• Understanding of chronic illnesses, advanced illnesses, and the nuances of end-of-life care discussions.
• Strong computer skills to facilitate the documentation of patient information and usage of electronic medical records efficiently.

Compensation and Benefits

The compensation for this role ranges from USD 120,000 to 130,000 annually, complemented by a comprehensive benefits package that includes medical, dental, and vision insurance, a retirement savings plan, and paid time off.
