Work Environment

This position is situated in our outpatient office in Rochester, NY, where Nurse Practitioners work closely with a multidisciplinary team. Operating primarily during regular business hours, Monday to Friday from 9 AM to 5 PM, our facilities are designed to support effective collaboration and efficient patient care. Here, you will engage in thorough assessments and interventions while leveraging advanced medical technologies to enhance patient outcomes.

Key Responsibilities

• Conduct comprehensive health assessments to identify patient needs and develop appropriate care plans tailored to individual medical histories.
• Manage patients through acute medical challenges and exacerbations of chronic conditions, including diagnostic testing and therapeutic interventions.
• Oversee short-term rehabilitation initiatives for patients recovering from acute health episodes, facilitating focused support and resources.
• Document patient conditions accurately using ICD-10 coding standards to ensure seamless communication across the care team.
• Collaborate with physicians, nurses, and allied health professionals to implement interdisciplinary treatment plans effectively.
• Lead conversations on advanced illness management and care planning, ensuring patients and families understand their options.
• Provide ongoing education, counseling, and support to patients and caregivers, enabling them to manage health conditions effectively.

Patient Population / Client Focus

Our Nurse Practitioners will serve a diverse range of patients with various internal medicine needs, focusing on individuals with complex medical, behavioral, social, and functional challenges. With a deep understanding of chronic illness management, you will contribute to improving the quality of life for patients at different stages of their health journeys—fostering resilience and promoting well-being in the community.

Qualifications

• Active certification as a Nurse Practitioner and a current state nursing license.
• Master's degree in nursing from an accredited institution.
• A minimum of three years of clinical experience in a healthcare setting, preferably in a hospital or outpatient environment.

Preferred Experience

• Clinical experience in long-term care, skilled nursing, acute care, or urgent care settings is highly desirable.
• Background in managing the medical needs of patients with both behavioral and chronic health conditions.

Compensation and Benefits

We offer a competitive salary range of USD 110,000 to 125,000 per year, commensurate with experience, alongside a comprehensive benefits package.
