Work Environment

This position is based onsite at local schools, providing direct healthcare services to students and their families. You will work in a collaborative environment that fosters professional growth, team synergy, and community health improvement. Your average work week will consist of approximately 40 hours, with a typical schedule running from 9:00 a.m. to 6:00 p.m., along with occasional evening clinics until 8:00 p.m. Participation in community outreach events or health fairs during weekends may also be required.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for students and their families, utilizing tools such as electronic health records for accurate documentation.
• Diagnose and manage a variety of acute and chronic medical conditions, with a focus on preventive care and health promotion strategies tailored to the community.
• Collaborate with local schools, organizations, and stakeholders to design and implement health education programs that address specific community health challenges.
• Provide culturally sensitive care and passionately advocate for patients' rights and access to health services, especially for underserved populations.
• Ensure continuity of care by coordinating referrals and follow-up for necessary treatments or specialty services, using established protocols to facilitate effective transitions.
• Maintain detailed and accurate medical records in compliance with healthcare regulations, ensuring confidentiality and data integrity.
• Engage in community outreach initiatives, such as health fairs and educational sessions, to raise awareness about health issues and empower individuals with knowledge.
• Stay actively updated with emerging community health trends, evidence-based practices, and relevant healthcare policies.

Patient Population / Client Focus

You will primarily work with students, their families, and community members within Pittsford, addressing a broad spectrum of health concerns that are prevalent in diverse populations. Qualifications

• Current certification as a Nurse Practitioner in the state of New York.
• Active state nursing license, ensuring compliance with state regulations and standards.
• Bachelor's degree in nursing; advanced degree in nursing is preferred.
• A minimum of 4 years of clinical experience, ideally in community health or public health environments.
• Exemplary interpersonal and communication skills, enabling successful collaboration with a range of stakeholders.

Preferred Experience

Compensation and Benefits

Jackson Medical Center offers a competitive salary range of $180,000 to $200,000 per year, commensurate with your experience and qualifications.
