Key Responsibilities

As a Nurse Practitioner at Support Individuals Healthcare Services, you will have a wide range of responsibilities, including but not limited to:

• Conducting comprehensive physical examinations of residents to assess their health and wellness.
• Diagnosing and treating acute, episodic, and chronic illnesses, tailoring treatment plans to individual patient needs.
• Prescribing necessary medications and formulating treatment regimens while ensuring adherence to best nursing practices.
• Developing and implementing patient management policies, procedures, and protocols to enhance patient care quality.
• Facilitating referrals to specialists and other healthcare professionals as needed to ensure seamless continuity of care.
• Maintaining open communication with collaborating physicians and specialists regarding patient care and management updates.
• Providing guidance and support to clinical staff to ensure effective patient management and care delivery.
• Participating in continuous education programs and pursuing opportunities to expand your professional expertise.

Patient Population / Client Focus

The primary patient population you will serve consists of senior residents and low-income individuals residing within our assisted living facility. These patients often present with complex medical histories requiring a holistic, patient-centered approach to care. Your role will be critical in not only addressing their medical needs but also understanding their psychosocial factors, enabling you to establish rapport and trust while providing dignified and compassionate care.

Qualifications

The ideal candidate for this Nurse Practitioner position should possess the following qualifications:

• A Bachelor’s Degree in Nursing, with advanced training through a Master of Science in Nursing (MSN), Doctor of Nursing Practice (DNP), or completion of an accredited Nurse Practitioner program.
• Active, unrestricted Nurse Practitioner licensure in New York State.
• DEA license permitting the prescribing of controlled medications.
• National certification through either the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
• Eligibility to work in the United States.
• A demonstrated passion for working with senior and low-income populations.

Preferred Experience

While not mandatory, candidates with the following experiences will be preferred:

• A background in geriatric care or palliative medicine, especially within assisted living or similar settings.
• Proficiency in electronic health record (EHR) systems, specifically in documenting patient assessments and care plans.
