Key Responsibilities

• Conduct thorough assessments and develop individualized care plans for residents with both acute and chronic medical conditions.
• Implement evidence-based treatment strategies in collaboration with physicians and other healthcare professionals to enhance patient outcomes.
• Order diagnostic tests, interpret results, and prescribe medications with a focus on individualized patient care.
• Educate residents and their families on disease prevention, medication management, and recovery processes to empower them in their health journey.

Patient Population / Client Focus

This role primarily serves an elderly population, many of whom may have complex health issues and varying levels of dependency. Qualifications

• Current certification as a Nurse Practitioner and maintaining an active New York state nursing license.
• A bachelor's degree in nursing with significant advanced training in clinical nursing.
• Minimum of 2 years of direct patient care experience, with a preference for experience in an assisted living or similar setting.

Compensation and Benefits
