Key Responsibilities

• Conduct thorough assessments and develop individualized treatment plans based on scientific rationale and established standards of care.
• Diagnose and treat acute healthcare problems including illnesses, infections, and injuries, ensuring timely interventions.
• Prescribe medications effectively, prioritizing safety, efficacy, and cost considerations, in line with legal authorization.
• Provide recommendations for lifestyle modifications to address health risks, facilitating better health outcomes.
• Educate patients and their families on the self-management of chronic illnesses, ensuring tailored instructions suited to their unique circumstances.
• Utilize Electronic Health Record (EHR) systems efficiently, maintaining accurate and complete patient records in accordance with policy regulations.
• Order, perform, and interpret diagnostic tests such as complete blood counts (CBCs), electrocardiograms (EKGs), and radiographs (X-rays).
• Analyze patients' medical histories and current health data to develop accurate diagnoses and appropriate treatment pathways.

Patient Population / Client Focus

The primary patient population consists of individuals living in assisted living facilities, each with unique health needs and varying levels of independence. As the Nurse Practitioner, you will be responsible for managing both acute and chronic conditions, focusing on maximizing the health, comfort, and dignity of our residents through individualized care plans.

Qualifications

• A valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA) is mandatory.
• A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
• Current certification as a Nurse Practitioner from the NYSED.
• A Bachelor’s Degree in Nursing or a related field is required.
• Demonstrated proficiency in utilizing Electronic Health Record (EHR) systems.

Preferred Experience

• Prior experience in assisted living or long-term care settings is advantageous.
• Experience in managing geriatric populations and chronic disease management will be prioritized.
• Ability to communicate effectively with patients, families, and multidisciplinary team members to develop cohesive healthcare plans.

Compensation and Benefits
