Position Overview

Riverhead Physical Medicine and Rehabilitation is seeking a dedicated and qualified Lead Nurse Practitioner specializing in Adult-Gerontology to join our dynamic healthcare team in Henrietta, New York. In this full-time, onsite role, you will engage in providing high-quality, value-based primary care tailored to meet the needs of adults aged 65 and older. Work Environment

The work setting is primarily located in an Assisted Living Facility, where you will operate within a supportive, multidisciplinary team, including physicians, physician assistants, medical assistants, registered nurses, and dedicated care managers. Together, this team approach allows for seamless care coordination, ensuring that every patient’s journey through the healthcare system is smooth and efficient. Our office hours are Monday through Friday from 9:00 AM to 5:00 PM, providing consistency for both staff and patients.

Key Responsibilities

• Deliver exceptional primary care services, focusing on preventive care and chronic disease management.
• Perform comprehensive assessments during office visits for both routine and acute health issues, ensuring thorough documentation and follow-up.
• Conduct annual wellness visits and health risk assessments while applying a holistic understanding of patient healthcare needs.
• Coordinate care with other healthcare providers, specialists, and testing facilities, streamlining patient access to necessary services.
• Lead population health initiatives, ensuring appropriate screenings and preventive care for all eligible patients.
• Engage in patient education regarding their health conditions, treatment plans, and wellness strategies, fostering a strong understanding and compliance.
• Participate in team outreach efforts, including phone triage to check-in on patients and address any emerging health concerns.
• Conduct home visits as required to ensure continuity of care for patients who may be homebound.
• Collaborate closely with the Medical Director and operational leadership to adhere to the organization’s goals and standards.
• Engage in community events and promotional activities to foster a healthy relationship between our organization and the community.

Patient Population / Client Focus

The primary patient population includes older adults who are often facing complex medical and social issues. Your role will be crucial in providing integrated care that acknowledges and addresses the multifaceted needs of our patients, promoting their well-being and enhancing their quality of life within their communities. We emphasize building ongoing relationships that facilitate trust, communication, and proper understanding of treatment plans.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or a Graduate of an NP Certificate Program.
• National certification as a Family Nurse Practitioner, Adult Nurse Practitioner, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license.
• Current, valid DEA license.
• Proven experience in primary care settings, preferably in internal medicine, geriatrics, or family medicine.
• Familiarity with HCC documentation, ICD-10 coding, and conducting health risk assessments.
• Strong commitment to teamwork and collaborative healthcare delivery.

Preferred Experience

• Previous experience working in assisted living or similar healthcare facilities.
• Experience with population health management and value-based care models.
• Proficiency with electronic health record (EHR) systems to enhance patient care delivery.

Compensation and Benefits

This position offers a competitive salary range of USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
