Key Responsibilities

• Conduct thorough physical examinations to assess the health status of residents and respond promptly to urgent medical needs.
• Diagnose and treat a range of acute and chronic health conditions, tailoring care plans to meet individual patient needs.
• Order, perform, and interpret various diagnostic tests such as blood tests, imaging studies, and other necessary evaluations.
• Develop and implement comprehensive treatment plans that may include prescribing medications, therapies, and lifestyle modifications.
• Administer treatments and medications with precision, ensuring adherence to best practices and regulatory standards.
• Maintain meticulous documentation of patient encounters and treatment plans using ICD-9 coding to ensure compliance and continuity of care.
• Collaborate closely with physicians, nurses, and other healthcare staff to create a patient-centered approach to care.
• Educate patients and their families on managing health conditions, promoting preventative care, and fostering healthier lifestyles.

Patient Population / Client Focus

The Nurse Practitioner will work primarily with residents in the assisted living facility setting, including older adults who often have complex health needs. This role requires a sensitive approach to care, as many patients may experience cognitive or physical impairments. Understanding the nuances of geriatrics and providing empathetic, respectful interactions is crucial for building trust and ensuring the best outcomes.

Qualifications

• A Bachelor’s Degree in Nursing and a valid Nurse Practitioner license.
• Minimum of two years’ experience in urgent care, geriatrics, or a related field to be eligible for this role.
• Proven ability to work autonomously as well as collaboratively in a multidisciplinary team.
• Effective communication skills, both verbal and written, with a focus on patient education.

Preferred Experience

• Experience with electronic health record (EHR) systems, particularly Cerner, to streamline documentation and maintain patient records.
• Knowledge in fields like cardiology, memory care, pediatrics, and geriatrics, as the role will involve interacting with a varied patient population.
• Ability to perform specific procedures, including but not limited to botulinum toxin treatments, is considered a valuable asset.
• Strong understanding of medical terminology, anatomy, physiology, and pharmacology to provide informed patient care.

Compensation and Benefits
