Parkland Health and Hospital PC is actively seeking passionate and dedicated Nurse Practitioners to enhance our Primary Care team in Rochester, New York. Work Environment

As an onsite role primarily focused on home care, you will engage with patients in their own living environments, providing a level of comfort and accessibility that enhances the quality of care. You’ll be supported by a multidisciplinary team in a collaborative atmosphere that values communication and teamwork. Your work schedule will consist of 40 hours per week, Monday through Friday, allowing for a work-life balance while achieving professional goals.

Key Responsibilities

• Conduct thorough physical examinations and evaluate medical histories to inform diagnostic processes.
• Order and interpret diagnostic tests such as blood work and imaging studies, ensuring accurate assessments of patient conditions.
• Diagnose health conditions and devise appropriate treatment plans for both chronic and acute illnesses.
• Prescribe medications and manage ongoing patient care, maintaining comprehensive documentation of treatment protocols.
• Educate patients and their families about health conditions, medication regimens, and preventive health practices.
• Promote and implement healthy lifestyle choices and disease prevention strategies through patient engagement and education.
• Participate in continuous professional development and research initiatives to remain current with best practices in nursing and medicine.

Patient Population / Client Focus

In this role, you will primarily focus on a diverse patient population with varying health needs. By providing care in a home setting, you will connect with individuals who may face barriers to accessing traditional healthcare facilities. This experience allows you to foster strong relationships, enhance patient compliance, and promote better health outcomes through personalized care plans tailored to each patient’s lifestyle and circumstances.

Qualifications

• A valid New York State Nurse Practitioner license.
• A Master's degree in Nursing or a related field.
• Certification as an Adult Nurse Practitioner (ANP) or Family Nurse Practitioner (FNP) is required.
• A valid DEA number is mandatory.
• Strong problem-solving skills, patience, and the ability to communicate effectively with a diverse patient base.
• Commitment to teamwork and collaboration within the healthcare setting.

Preferred Experience

• Experience working in primary care, urgent care, or emergency departments is highly favored.
• Previous exposure to chronic disease management or home care settings will enhance your candidacy.
