Work Environment

This position is full-time, with a structured work schedule typically ranging from 9:00 a.m. to 6:00 p.m., including the possibility of evening clinics until 8:00 p.m. You may also participate in outreach activities during weekends to promote community health initiatives. The role is onsite, allowing you to engage directly with patients in their home settings, which fosters a stronger provider-patient relationship.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, addressing their specific health concerns and needs.
• Diagnose a wide range of acute and chronic medical conditions, tailoring treatment plans that incorporate preventive care and health promotion strategies.
• Collaborate with local organizations and behavioral health clinicians to design and implement community-focused health education programs.
• Advocate for patients' rights and ensure culturally sensitive care, providing guidance and support for seamless access to healthcare services.
• Coordinate patient referrals and manage follow-up care, ensuring continuity and comprehensive healthcare for each patient.
• Maintain meticulous and compliant medical documentation that adheres to regulatory standards and facilitates efficient care transitions.
• Engage in community outreach events and health fairs, promoting health awareness and offering critical health screenings to underserved populations.
• Stay current with developments in community health trends, best practices, and relevant policy changes, integrating new knowledge into your practice.

Patient Population / Client Focus

You will primarily serve a diverse population of patients in their homes, emphasizing those who may face barriers to accessing traditional healthcare settings. This includes elderly individuals, families with chronic health issues, and underserved communities. Your work will directly contribute to improving their health literacy, empowerment, and overall quality of life.

Qualifications

• A Master's or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized credentialing bodies.
• A valid, unrestricted Nurse Practitioner (APRN) license in New York State, along with an active DEA and NPI number.
• Strong clinical assessment and diagnostic skills, with a commitment to delivering exceptional patient care.
• Excellent communication and interpersonal skills to effectively interact with patients and team members.
• Ability to work autonomously while also collaborating well as part of a multidisciplinary healthcare team.
• Familiarity with Electronic Health Records (EHR) systems.

Preferred Experience

• Minimum of three to five years of clinical experience, preferably in a primary care, family medicine, community health, or public health setting. Experience in home care will be advantageous.
• Proven ability to work with diverse populations, demonstrating cultural competence and sensitivity in your approach.

Compensation and Benefits
