Work Environment

This position is based onsite at our well-established hospital facility, where you will collaborate closely with a skilled and supportive interdisciplinary care team that includes physicians, nurse practitioners, physician assistants, medical assistants, and a dedicated medical scribe. Key Responsibilities

As an Adult-Gerontology Nurse Practitioner, your responsibilities will include:

• Providing outstanding primary care tailored to the specific needs of adult patients, incorporating both routine and acute care.
• Conducting comprehensive office visits assessing patients’ health concerns, addressing acute issues, and following through on preventative care.
• Administering annual wellness visits coupled with health risk assessments, ensuring meticulous documentation that holistically captures patient health.
• Coordinating care with other providers, specialists, testing facilities, and community agencies, contributing to streamlined patient experiences.
• Championing population health initiatives by leading efforts to ensure patients receive necessary screenings and preventive services, such as biannual breast cancer screenings for eligible females.
• Participating in phone triage and outreach, ensuring that patients receive timely support between appointments.
• Educating patients and their families on health conditions, treatment options, and care strategies, enhancing their understanding and engagement in health management.
• Engaging in promotional activities that raise awareness of our services and the importance of preventative care.
• Performing home visits when necessary to provide continuity of care to patients who may need additional support.
• Reporting directly to the Medical Director while collaborating with operational leadership to enhance our care delivery systems.

Patient Population / Client Focus

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through the AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license.
• A current DEA license to prescribe medications.
• Experience in primary care settings, specifically internal medicine, geriatrics, or family medicine.
• Proficiency in HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A strong preference for teamwork, with a collaborative spirit to work effectively with various healthcare professionals.

Preferred Experience

While not mandatory, familiarity with advanced care planning and a solid understanding of value-based care principles will be considered advantageous. Previous experience in a quality improvement initiative or population health leadership roles is also a plus.

Compensation and Benefits

This full-time position offers a competitive annual salary ranging from USD 120,000 to 160,000, commensurate with experience and qualifications.
