Work Environment

This full-time position is onsite, predominantly within clients' homes in Henrietta, New York, offering a unique opportunity to engage with patients in their personal settings. Key Responsibilities

• Facilitate the admission, transfer, and discharge processes for patients, coordinating necessary home care and other accommodations.
• Engage in comprehensive patient assessments by performing detailed medical, surgical, and psychosocial histories, documenting findings accurately.
• Conduct phone triage and assessments to address acute clinical concerns, prescribing treatment protocols in line with established guidelines.
• Collaborate with multidisciplinary teams, directing and delegating patient care activities that encompass teaching, counseling, and discharge planning.
• Assist in developing and evaluating care standards, ensuring compliance with institutional policies and health regulations.
• Order and interpret diagnostic studies such as laboratory work and imaging, utilizing clinical data to inform treatment decisions.
• Perform other related duties as necessary to support patient care and operational efficiency.

Patient Population / Client Focus

Your role as a Nurse Practitioner will involve a diverse patient population from various socioeconomic backgrounds. You will work with individuals who require home care services, including those with chronic illnesses, recovery from acute conditions, and post-operative care, ensuring that each patient receives personalized treatment and education about their health conditions.

Qualifications

• Current NP Board Certification (FNP or PNP), reflecting competency in family or pediatric nurse practitioner standards.
• A valid DEA License, enabling the prescription of necessary medications.
• Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications to ensure preparedness for emergency situations.
• Proficiency in using electronic health record (EHR) systems and technology for patient documentation and care coordination.
• Strong organizational skills with an attention to detail, essential for managing patient care effectively.
• A current New York State NP License and RN License, required to practice in the state.

Preferred Experience

Experience in home care, chronic disease management, or acute care settings is highly advantageous. Previous work collaboratively with interdisciplinary teams will enhance your ability to deliver comprehensive care seamlessly. Familiarity with the use of telehealth systems for virtual consultations may also be beneficial in this role.
