Work Environment

This position is onsite and involves working closely with patients in a home care setting. You'll be providing treatments and follow-ups in the comfort of our patients' homes, allowing for a personalized approach to their care. Key Responsibilities

• Conduct thorough patient assessments to diagnose and manage a variety of cardiology conditions such as hypertension, heart failure, and arrhythmias.
• Develop and implement tailored treatment plans in collaboration with the healthcare team, ensuring a patient-focused approach that integrates the latest clinical guidelines.
• Utilize advanced diagnostic tools and techniques—including echocardiograms and stress tests—to evaluate cardiovascular health and make informed treatment decisions.
• Monitor patient progress through regular follow-up appointments, adjusting treatment plans as needed to achieve optimal health outcomes.
• Provide critical education to patients and their families regarding managing their cardiovascular diseases, including medication adherence, lifestyle changes, and ongoing monitoring.
• Collaborate with other healthcare professionals, including cardiologists and primary care physicians, to deliver holistic and inter-disciplinary patient care.

Qualifications

• A Master of Science in Nursing (MSN) degree with a focus on cardiology or a Family Nurse Practitioner program that encompasses cardiology coursework.
• Certification as a Cardiology Certified Nurse Practitioner (CCNP) by the Cardiology Nursing Certification Board (CNBC).
• A minimum of two years of relevant experience working in cardiology, primary care, or a related field as a Nurse Practitioner.
• Demonstrated clinical expertise, strong organizational skills, and the ability to communicate effectively with both patients and team members.

Preferred Experience

Compensation and Benefits
