Work Environment

Key Responsibilities

• Engage in community outreach initiatives, participating in health fairs and events to promote cardiovascular health awareness and provide free health screenings.
• Coordinate and manage referrals alongside follow-up care, ensuring seamless transitions for patients within the spectrum of care.
• Deliver culturally competent care that respects patients’ rights and advocates for equitable access to healthcare services.
• Conduct thorough health assessments and screenings, employing evidence-based practice to identify both acute and chronic medical conditions.

Qualifications

• A Master’s degree in nursing from an accredited program.

Compensation and Benefits
