Work Environment

The position is primarily onsite at our assisted living facility in Union Hill, where the Nurse Practitioner will be part of a multidisciplinary team. Key Responsibilities

• Conduct in-depth evaluations of patients' cardiovascular health, including physical examinations and detailed patient histories.
• Develop and execute personalized treatment plans based on patient evaluations, incorporating evidence-based practices and guidelines specific to cardiology.
• Monitor and manage ongoing cardiac conditions, adeptly adjusting treatment strategies as needed, and utilizing electronic health record systems to document care plans.
• Administer and interpret various diagnostic tests such as EKGs, echocardiograms, and stress tests, providing timely communication of results to patients and their families.
• Manage medication regimens, ensuring patients understand their prescriptions and any potential side effects associated with cardiac medications.
• Educate patients and their families about heart-healthy lifestyle choices and disease management strategies to improve overall health outcomes.
• Act as a liaison between patients and other healthcare providers, making appropriate referrals to specialists when necessary.

Patient Population / Client Focus

This role will primarily focus on residents of the assisted living facility who are at risk of cardiovascular diseases or already living with such conditions. The position requires a strong understanding of the unique challenges faced by this population, including age-related health issues, chronic disease management, and the psychological impact of living with cardiac conditions. Providing culturally competent care and establishing trust with patients will be essential for successful health outcomes.

Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license in New York.
• A Bachelor's degree in nursing with advanced training through a Master's degree in nursing from an accredited program.
• At least two years of clinical experience as a Nurse Practitioner, preferably with a specialization in cardiology or experience in environments focusing on community and public health.

Preferred Experience

• Experience working within assisted living facilities or with geriatric populations is strongly preferred.

Compensation and Benefits
