Key Responsibilities

• Order and interpret lab and diagnostic tests, utilizing results to provide effective treatment plans tailored to individual patient needs.
• Maintain comprehensive electronic medical records (EMR) to ensure compliance with state and federal regulations and facilitate continuity of care.
• Collaborate with local and regional leadership to meet organizational goals, focusing on strategic initiatives to enhance value-based care.
• Provide exceptional customer experiences, fostering high satisfaction and retention rates through attentive, empathetic service.
• Adhere to ethical practice standards while following the Scope and Standards of Practice outlined by the profession and relevant boards.
• Engage regularly with primary care providers and interdisciplinary care teams to coordinate comprehensive care plans.
• Conduct monthly, quarterly, and annual visits to monitor and manage both acute and chronic conditions as necessary.
• Lead family conferences and provide education on advanced care planning, ensuring that families understand the options available.
• Collaborate with pharmacists to address medication recommendations and ensure adherence to prescribed therapy.
• Conduct thorough assessments and develop detailed treatment plans, clearly communicating these to patients, families, and other care providers.

Patient Population / Client Focus

Your primary focus will be on elderly patients as well as low-income populations who may face unique healthcare challenges. You will be responsible for addressing their medical needs with empathy and understanding, ensuring that every patient receives personalized care that empowers them to achieve their health goals.

Qualifications

• Must hold an active, unrestricted state Nurse Practitioner license in New York.
• Possess an active DEA license for the prescription of controlled substances.
• Valid work authorization to practice in the United States.
• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited NP Certificate Program is required.
• Certification from the American Association of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) is required.

Preferred Experience

• A minimum of two years of experience working as a Nurse Practitioner, preferably in a home care or geriatric setting.
• Strong understanding of the challenges facing senior patients and low-income groups in accessing healthcare.
• Demonstrated ability to work independently in a flexible environment while maintaining high standards of care.

Compensation and Benefits

The compensation for this full-time position ranges from USD 150,000 to 160,000 annually, commensurate with experience.
