Position Overview

Mountain Park Pulmonology PC is actively seeking a dedicated and skilled Nurse Practitioner (NP) with a specialized focus in Cardiology to join our dynamic outpatient practice located in Penfield, New York. This full-time role is pivotal in delivering high-quality cardiac care within the Center for Advanced Cardiac Care (CACC), working alongside a team of experienced healthcare professionals to enhance patient outcomes.

Work Environment

The position is based onsite in our state-of-the-art outpatient office, where our team operates Monday through Friday from 8 AM to 5 PM. There are no overnight calls or weekend shifts, promoting a work-life balance that is crucial for healthcare professionals. The office is designed to foster collaboration among team members, providing an environment where Nurse Practitioners work closely with Cardiologists, Registered Nurses, and administrative staff to improve the overall patient experience.

Key Responsibilities

• Conduct comprehensive patient assessments, including medical history and physical exams, to formulate individualized care plans.
• Collaborate with Cardiologists to manage a diverse patient caseload, focusing on conditions such as Hypertension, High Cholesterol, Coronary Artery Disease, and Congestive Heart Failure.
• Administer outpatient-only Electrophysiology services, including evaluating patients with abnormal electrocardiograms, managing device patients, and providing education on arrhythmias.
• Perform Stress Testing as part of the cardiac evaluation while ensuring high standards of patient safety and care.
• Facilitate referrals to specialists within the cardiac care team as needed, ensuring a seamless continuum of care.
• Educate patients and their families about disease management, treatment options, and preventive measures to improve heart health.
• Utilize EMR systems for documentation, ensuring compliance with healthcare regulations and facilitating coordinated care.

Patient Population / Client Focus

The Nurse Practitioner in this role will be primarily working with adults experiencing cardiovascular issues. Our patient population spans various demographics, with conditions ranging from chronic diseases such as coronary illnesses to acute presentations requiring immediate and skilled management. Understanding the unique needs of each patient is essential in developing effective treatment strategies that lead to improved health outcomes.

Qualifications

• A Bachelor’s degree in Nursing with an accredited Nurse Practitioner program completion.
• Current and valid Nurse Practitioner license.
• Certification in a relevant specialty area is strongly favored.
• Ability to assess, diagnose, and treat various cardiovascular conditions while providing exemplary patient care.

Preferred Experience

We welcome applications from both seasoned Nurse Practitioners and newer graduates. Candidates with prior experience in an inpatient or outpatient Cardiology setting will be highly regarded. Compensation and Benefits
