Work Environment

This role is primarily based onsite at an assisted living facility in Henrietta, New York. You will be part of a multidisciplinary team that collaborates closely to optimize patient care. The facility is designed to promote independence while offering comprehensive support for residents, many of whom have complex healthcare needs, particularly concerning cardiovascular health.

Key Responsibilities

• Conduct thorough cardiovascular assessments for patients, including obtaining medical histories, performing physical examinations, and interpreting diagnostic tests such as EKGs and echocardiograms.
• Develop and implement personalized treatment plans, prescribing appropriate medications and therapy to manage both acute and chronic cardiac conditions.
• Educate patients and their families about cardiovascular health, including lifestyle modifications and preventive care strategies tailored to their individual risks and health goals.
• Maintain accurate medical records in alignment with healthcare regulations, ensuring that all patient interactions and treatment plans are documented following best practices.
• Engage with community organizations to design and implement heart health education programs, fostering awareness and proactive management of cardiovascular risks.
• Advocate for patients, ensuring they have access to necessary cardiology services and supporting their rights throughout their healthcare journey.
• Stay current with trends in cardiology through ongoing professional development and collaboration with colleagues in the healthcare industry.

Patient Population / Client Focus

The patients you will serve are primarily residents of an assisted living facility, often facing various healthcare complexities related to aging, lifestyle, and pre-existing health conditions. Qualifications

• A Master’s degree in Nursing from an accredited program, with a focus on advanced clinical practice in cardiology or a related field.

• A minimum of three years of clinical experience in cardiology or cardiovascular health settings, showcasing your ability to build patient rapport and make clinical judgments.

Compensation and Benefits

The compensation package for this position ranges from USD 150,000 to 160,000 annually, commensurate with experience. In addition to competitive salary, we offer:
